About our Financial Policy
Referral: It is the patient’s responsibility to ensure that the insurance referral is in place if it is required by your insurance carrier. Please be aware of your particular requirements and benefits associated with your policy.
Proof of Insurance: If you fail to provide us with your correct insurance card and information, you will be considered self pay and will be held responsible for the balance.
Insurance co-payments: All co-payments are due at the time services are rendered.
Insurance non-participating: If we are not participating with your insurance company, you will be responsible for the complete balance your insurance company requires.
Insurance authorization: I authorize direct payment of my insurance benefits to Dr. Gretchen S. Beck (DBA Blue Ridge Allergy & Asthma) for professional services rendered.
Self Pay Patients: If you are a self pay patient you will be required to pay a minimum of $200.00 on your initial visit and a minimum of $50.00 on all follow up visits. You will be required to arrange a payment plan at the time of your initial visit to our office.
Payment: We accept cash, checks, Visa and Master Card.
Returned checks: In addition to the face value of the check, we will bill you a return check fee of $30.00
Billing: Blue Ridge Allergy & Asthma will submit your claims and assist you in getting your claims paid promptly. if your insurance changes, it is your responsibility to notify our office. By being seen in our office, you accept responsibility for payment of all charges incurred, as well as all collection agency costs and/or attorney fees should such collection action become necessary.
